IF THE FORMS ARE NOT CORRECTLY COMPLETED, AND YOU BRING THE WAIVERS FOR SOMEONE ELSE’S CHILD TO YOUR APPOINTMENT,
WITHOUT THE LEGAL GUARDIAN PRESENT,
THEN WE WILL HAVE TO REFUSE THAT CHILD A RIDE AT THAT TIME.
